          Reporting Requirements: 

The completed match reports along with any supplemental/discipline reports should be faxed to 312-808-1301, Attn: Development Academy.

Alternatively, they can be scanned and sent to AAcademy@USSoccer.org

           Academy Game Report (Blank) (PDF) 

           IRS W-9 (PDF)  Download and complete to receive payment from US Soccer. 

              Scan & Email completed W-9 form to: Academy@USSoccer.org or fax to 312-808-1301 (Attn: Academy-Melissa Biniewicz)
